Insurance Information

Many insurance plans cover nutrition visits.We currently accept Blue Cross Blue Shield, Independence Administrators, AmeriHealth, Cigna and Aetna. Please call your insurance plan to verify that you have nutrition benefits prior to your visit.

If you have an HMO plan or POS plan which requires referrals, please request a referral from your PCP prior to your visit.

Your First Appointment

Please bring the following completed forms to your first appointment:

• Total Wellness Services Registration Form
• Nutrition History Form
• Food Diary Form

 Cancelation Policy

We will ask you for a credit card to hold your nutrition visit. Your card will not be charged unless you no-show for an appointment or do not call to cancel at least 24 hours ahead of time. This applies for initial and follow up visits.
